Personal Item:
Why is your character here: What it says on the tin. What did you need to escape? What did you want to change? Just show you’ve thought about why they’d come here! Especially important for people on the skill loss track, as they’ll know why they came.
Character history: A blow-by-blow playback of everything important ever that happened to a character is not only not necessary, it is not allowed. Keep it short, we just need a brief summary. Aim for 300 words.
Character personality: If you’re applying with the amnesia option, this doesn’t have to be much longer than your history, but for skill-route characters, this should be the part where you analyse your character, talk about their motivations, why they are the way they are, general behavior, ect! Show us you know the character you’re applying for.

Skill loss or memory loss?
And then, fill out one of the following, depending on which is applicable:
Amnesiac Personality: For amnesiacs, this is the focus of your app. Since you did the basics up above, NOW is the time to focus on how you’ll be playing them here, and more importantly WHY you’ll be playing them like that. Talk about traits that change or stay the same, WHY these traits are retained even without memories to shape their actions, or why they’re not.
Effect of losing skills: This is less of a big deal, but we still want it covered! If your guy is mister godmod in canon and suddenly he can’t kill everything with his pinky is he still going to attempt to do that, just get ground face down into the dirt repeatedly, is going to make them bitter, are they going to focus on learning new skills because trying and failing at things they should be able to do is too frustrating? Tell us about it! Things like MY VAMPIRE SUDDENLY HAS TO DEAL WITH EATING FOOD!! also go here, and if you capped anything like emotion mention that as well.
Effect of powercap: Both memory loss and skill lost must answer this question. Information on the powercap can be found at the bottom of the rules post, here. Please briefly address how your character's skills will be affected by it.
